SWCAP Neighborhood Health Partners
Neighborhood Health Partners is one of the many programs of the Southwestern WI Community Action Program www.swcap.org. Our program was initially established in 1973 as Southwest Family Planning and later as SWCAP Reproductive Health Care Center to provide services in Grant, Iowa, and Lafayette Counties. We added Green County in 2009. In 2012 we changed the name of our program to SWCAP Neighborhood Health Partners to better reflect our commitment to the communities we serve and our partnership with other resources for our patients.
The mission of Neighborhood Health Partners is to provide confidential and affordable reproductive health care to women and men of all ages in an atmosphere that promotes the dignity and respect of the individual while providing the education needed to make informed choices regarding their sexual health.
We provide primary preventative health care services for all people regardless of income, age, race, gender, sexual orientation, lifestyle, marital status, religion, or physical and mental abilities. There are convenient day and evening appointments to fit everyone’s schedule and we are handicapped accessible. We can also provide an interpreter with advanced notice.
